You’re right, it does stick down a little, but it’s barely noticeable. The extra ride height helps, absolutely, and the tire is ever so slightly rakes up towards the hitch. Here’s a pic from today after a snowfall.

I installed mine this weekend - the actual headliner wasn’t even touched. Maybe bent back a bit with the ratchet extension after the A, B, C pillars/panels have been moved or removed, to find the 4 nuts which hold the roof trim in place. But I did not remove anything from the headliner itself...
I have a Rockwood GeoPro 20BHS and tow with my RR great. No issues whatsoever. If I’m not mistaken, the RR has a lower tow rating, too. Only modifications I have is rear airbags to level the truck out, and a shocker hitch (Here) to keep my passengers happy.
