Correct that the lift is not 1:1. A 1.5" spacer gives around 2" of lift on the RR. But I believe that is more due to geometry than any significant load being put into the spring. The strut is not perpendicular to the ground, otherwise it would be closer to 1:1. I'd guess the rear is.

Top spacers do change the angle of the drive shafts and steering tie rods. Those can have some negative effects but if the spacer is not too big, it shouldn't be too noticeable. This will apply to either type of lift.
Wow. Run away from that shop. They don't have a clue what they are talking about. Only perch spacers add more preload to the spring, which in terms gives a rougher ride. And the shock is not sitting in its normal rest position. A top spacer lift doesn't affect any of those.
I'm betting it is a different airbag with the handle. Pretty sure the mounting points are not on ours and would have to be welded, in addition to a different airbag that may or may not have the same connector.
